Why communities? 

Health systems everywhere are struggling with the same wicked challenges.  While the contributing causes across these systems are similar, the actual combination of factors and the relative influence of each varies widely from context to context.  
This means there's no "one size fits all" approach for solving these problems.  But the people who regularly interact with each system hold the solution for that system.  And that solution lies hidden within the stories they're already sharing with others about their day-to-day experiences.
This is why ShareTeam creates communities from the people regularly interacting with the systems we target for support.  We gather their stories and start exposing meaningful patterns hidden within those stories.  We engage the community to produce continuous learning.  We leverage that engagement to facilitate meaningful action.  And we monitor and report on the impact of those incremental actions to produce powerful feedback loops.
